To request ukuleles for your music therapy program, please complete the form below. Once submitted, your site will be added to our donation list.
How It Works

- Our team will review your submission and add your site to the fulfillment list.
- Most ukulele donation requests are fulfilled within 15 to 30 days.
- Once your donation is ready to ship, our team will follow up with more information.

UKC Partner Responsibilities
As a UKC music therapist and partner program, your main responsibility is simple: Continue doing the incredible work you already do by sharing the healing power of music with children and families. We also request that you:
- Complete our donation tracking form each time a ukulele is gifted to a child. This helps us ensure donations are reaching the right places and allows us to share accurate impact reports with our donors and supporters.
- Â Share photos and videos of the donations when you request more ukes, Â whenever possible and with appropriate family consent. While never required, these stories and moments help bring the mission of UKC to life and inspire continued support our mission.
- Join UKC at Home and share with the kids. This program offers continuing education opportunities, music therapy resources, therapeutic ukulele materials, and virtual support services for children and families.
